Police were called to the crash on Grand Promenade, near Catherine Street in Bedford about 8.50am on Sunday.
A white BMW and gold Holden Astra, travelling in opposite directions and separated by a median island, collided in the north bound lanes on Grand Promenade.
St John Ambulance attended the scene and rushed three people under priority conditions to Royal Perth Hospital.
The driver of the BMW, a 46-year-old man, escaped serious injury but his passenger, an 18-year-old man, has injuries believed to be life-threatening at this time.
The driver of the gold Holden Astra, an 85-year-old woman, also has serious injuries believed to be life-threatening at this time.
Major Crash Investigators are seeking information on the crash.
Later in the morning, two people were believed to be trapped in a car after a crash in Mt Hawthorn.
Emergency services crews were called out to Oxford Street after the vehicle rolled. The crash happened about 11.40am near the corner of Anzac Road.
Police, St John Ambulance and fire crews attended.
